on the u.s. consulate in benghazi, libya, has issued a scathing report, finding what it calls schematic failures at the state department leading up to the tack, which took the lives of chris stevens and three other americans. abc's martha raddatz, who has read the report this morning, has the details. >> reporter: good morning, josh. the report calls the security in benghazi grossly inadequate, despite repeated requests for increased staffing. the report also cites poor leadership win the diplomatic department. saying certain senior state department officials demonstrated a lack of proactive leadership and management ability in their response to the security concerns in benghazi. hillary clinton has already responded to the independent review, saying she will adopt all 29 recommendations, including additional marine corps security guards. but there's one conclusion that also stands out. the board determined that those u.s. personnel on the ground in benghazi performed with courage and willingness to risk their own lives to protect their colleagues in an impossible situation. josh? >> ce
